Dr. Nelson does not accept insurance or work directly with insurance companies or other third-party payers. Sometimes her services are covered by your insurance policy even though she is an "Out of Network" provider. We recommend you check with your insurance company’s Summary of Benefits to see if you have Out of Network benefits as part of your insurance plan.
Payment in full is due at each appointment.
We can provide a Super Bill (an itemized receipt with a diagnosis code) for you to file with your insurance company.
You can fax a Super Bill to your insurance company with their filing form.

Please provide 24 hour notice for a cancelation or to reschedule your appointment. Failure to do so will result in a fee of 50% of agreed upon rate for that service
A Good Faith Estimate provides the client with an estimate of out of pocket costs for services provided by Dr. Nelson based on the information known at the time the estimate is created. Services include your initial intake appointment, relevant screening tools and interpretation, diagnosis, counseling and medical management if indicated. Length of treatment is difficult to estimate initially as it depends on if the issue is acute or chronic and if it is managed with medication. If you do not plan to submit a superbill for reimbursement, a GFE will be provided based the best estimate with the information at hand.
